K21 Health Foundation exists for the benefit of Kosciusko County citizens to ensure health care services are provided, and to advance prevention and healthy living. This will be accomplished by identifying health needs in our community, and maintaining an endowment so funding is available, through investments and grants, for those needs.

When submitting an application, your organization must clearly explain:

  • How supporting your project or program will advance K21’s mission
  • What the intended benefit(s) are for residents of Kosciusko County

We want to hear your idea before you submit an application. The first step in the grant request process is to complete a Pre-Grant Questionnaire (LOI). This questionnaire is the first form you will complete in the grant portal. Once the LOI is submitted, K21’s Grants Manager will connect with you to either open the application form or schedule a meeting to discuss your idea.

During a pre-application meeting, our Grants Manager will discuss your ideas with you, help you determine whether or not your project fits K21’s requirements, and provide guidance on which kind of grant you should apply for and how to word your request appropriately.

K21 Health Foundation accepts grant applications four times each year.

2026 Deadlines are:

Quarter 1

  • LOI – January 19
  • Application – February 2

Quarter 2

  • LOI – April 17
  • Application – May 1

Quarter 3

  • LOI – July 17
  • Application – July 31

Quarter 4

  • LOI – October 19
  • Application – November 2

Pre-Grant Questionnaire and Application submissions are due by 5:00 pm on the dates listed.

To qualify for a grant from K21, your organization must meet one of the following requirements:

  • A non-profit agency with current IRS tax-exempt status and in good standing with the Indiana Secretary of State
  • A school system serving students of Kosciusko County
  • A governmental agency serving residents of Kosciusko County

To qualify for a grant from K21, your request must meet the following requirements:

  • The project or program you are seeking funding for must benefit the residents of Kosciusko County.
  • The impact must address health and wellness issues for our community.
  • The priorities for funding consideration are defined by our Health Pillars:
    • Community Culture & Wellbeing
    • Direct Services & Prevention
    • Health Access & Opportunity
    • Youth Development & Activity
