K21 Health Foundation exists for the benefit of Kosciusko County citizens to ensure health care services are provided, and to advance prevention and healthy living. This will be accomplished by identifying health needs in our community, and maintaining an endowment so funding is available, through investments and grants, for those needs.
When submitting an application, your organization must clearly explain:
- How supporting your project, program, or service will advance K21’s mission
- What the intended benefit(s) are for residents of Kosciusko County
Prior to submission, we encourage all applicants to set up a meeting with K21’s Grants Manager. Our Grants Manager will discuss your ideas with you, help you determine whether or not your project fits K21’s requirements, and provide guidance on which kind of grant you should apply for and how to word your request appropriately.
K21 Health Foundation accepts applications for grants four times each year. Application deadlines are:
- February 1
- May 1
- August 1
- November 1
Applications are due by 11:59pm on the dates listed, regardless of whether that day falls on a weekday or weekend. Applications received after these deadlines will be held until the next cycle.
