At K21 Health Foundation, our mission is to build a healthier, more vibrant Kosciusko County—now and for generations to come. We partner with organizations that strengthen health and wellbeing in our community. Our Health Pillars outline the types of opportunities we fund and how they contribute to fulfilling our vision for a healthier community.
- Community Culture & Wellbeing
- Direct Services & Prevention
- Health Access & Opportunity
- Youth Development & Activity
In partnership with DreamOn Studios, we created a five-video series that deepens understanding of our mission. The overview video presents a shared vision that guides our grantmaking, while each Health Pillar video dives deeper into a specific focus area, showing how K21’s strategy translates into real-world impact. Together, these videos tell a larger story about how K21 Health Foundation approaches community health—with intention, collaboration and a long‑term view.
Overview Video
Community Culture & Wellbeing
K21 Health Foundation supports nonprofit organizations, programs and projects that encourage a community culture to prioritize health. From developing and improving community infrastructure and amenities, to protecting and enhancing natural assets, to providing opportunities that support an active lifestyle—K21 is dedicated to building a health-minded community.
Direct Services & Prevention
K21 Health Foundation supports nonprofit organizations, programs and projects that provide direct health services and develop healthy habits for our residents. Community members who receive the care they need, when they need it, and learn behaviors they can apply toward lifelong health contribute to the wellbeing of our county.
Health Access & Opportunity
K21 Health Foundation supports nonprofit organizations, programs and projects that address barriers that prevent our community members from living healthy lives. These opportunities are created for residents to overcome economic, physical, cultural and social challenges that inhibit healthy behaviors, including homelessness, addiction recovery, abuse and more. When people have health access and opportunities, they take the first step toward a healthy lifestyle.
Youth Development & Activity
K21 Health Foundation supports nonprofit organizations, programs and projects that promote positive youth development and create opportunities for them to be active. Their ability to overcome barriers to development, address health issues that could hinder their progress and access preventive learning sets the stage for the rest of their lives. Youth who learn to live an active and healthy life become adults who prioritize their wellbeing.
“By offering greater clarity around our focus areas, we hope to empower organizations to better connect their projects, programs and initiatives with K21’s mission,” explained Rich Haddad, President & CEO of K21 Health Foundation. “When our grantee and community partners understand the Health Pillars, everyone benefits—especially through stronger, more focused grant requests that reflect our shared goals.”
